  Please note that the payment cheque/account/pay order to the Merchant should always be in the name of the club, association, society, charitable organization.

  4. Certified copy of registration documents/certificates, By-laws/Rules and Regulations forwarded by the Authorized Signatories under covering letter.
  5. An undertaking signed by all authorized person on behalf of the institution
     1. List of Members of the Governing/Executive/Managing/Working Committee
     2. It should be ensured that the individuals who are authorized and members of the governing body should be not affiliated with any prescribed/banned entity, whether under the same name or different name.
     3. Any other documents as deemed necessary including its annual accounts/financial statements or disclosures in any form which may help to ascertain the detail of its activities, sources and usage of funds in order to assess the risk profile of the prospective customer.
